Profile - Quick access to your member ID number, PCP information, group number and plan name.
Benefits Overview - Check your copays and coinsurance amounts so you know how much you will pay on your next doctor visit.
ID Card - View your current card or order a replacement card.
Deductible Summary - Obtain current information regarding amounts you have paid toward your deductible and your out-of-pocket amounts.
Visits and Claims - Obtain information regarding your latest claims, including status, claim number, provider and amounts paid.

The CommunityCare Connect mobile app is an easy way to access and manage your health insurance information. In addition to a tablet version of the app, here’s what you’ll find with the 2.0 release:
• You can enable Face ID, fingerprint or pin code for quick authentication (depending on your device).
• Access to the online provider directory from the app.
• A new 24/7 care tab with a link to Saint Francis MyChart.
• A link to the Price Comparison Tool.
• A Health & Wellness tab with a link to a personal health risk assessment and free wellness resources.
• A secure email feature to initiate a conversation with a CommunityCare Customer Service team member. You can expect a call within 24 hours.
• Updates to the Benefits Overview, Deductible Overview and Visits & Claims tabs to display all medical and pharmacy copays, accumulators and claim information for active plans.
• Improved navigation throughout the app.
